His mistake: He was climbing in crystal-ice conditions which contributed to a slip
What happened: World-renowned mountaineer, Roby Piantoni, was with a group attempting a new route up the south face of Shisha Pangma, the world’s 14th tallest mountain and the last 8,000-meter peak to be climbed.

The team attempted the same route days before but they were turned around due to high winds. They stayed at base camp in hopes to get another summit push. On the second attempt, the team found crystal-ice conditions. During the ascent, Piantoni slipped on the ice and fell to his death.
Piantoni, a professional Italian mountain guide has climbed to the summit of GII and Everest without oxygen.
